Alkoholbedingte Tumoren und genetische Suszeptibilität in Europa (ARCAGE)

Beschreibung

Jedes Jahr erkranken in den 27 Staaten der EU ca. 160.000 Menschen an Tumoren der oberen Luft- und Speisewege, ca. 90.000 sterben daran. Diese Erkrankungen umfassen Tumoren der Mundhöhle, des Rachens, des Kehlkopfes und der Speiseröhre, die damit zusammengenommen die vierthäufigste Krebserkrankung in der EU darstellen. Die von der International Agency for Research on Cancer (IARC, Lyon) koordinierte multizentrische Studie soll Gene identifizieren, die das Risiko für diese Erkrankungen beeinflussen, und diese hinsichtlich ihrer Wechselwirkung mit Lebensstilfaktoren – insbesondere Alkoholkonsum und Rauchen – untersuchen. Damit sollen vor allem zwei Ziele erreicht werden. (1) Bei alkoholischen Getränken und Tabakrauch handelt es sich um komplexe Stoffgemische, deren karzinogener Wirkmechanismus erst unvollständig geklärt ist. Diese Studie soll das Verständnis des karzinogenen Prozesses verbessern, indem untersucht wird, welche spezifischen Metabolisierungsenzyme oder Tumorsuppressorgene mit den Erkrankungen in Zusammenhang stehen. (2) Die Identifizierung von genetischen Polymorphismen, die mit der Entstehung von Krebs zusammenhängen, könnte genutzt werden, um Personen mit extrem erhöhtem Erkrankungsrisiko zu identifizieren, z.B. Raucher, die aufgrund ihres genetischen Profils für die Metabolisierung von Schadstoffen aus Tabakrauch eine erhöhte Suszeptibilität aufweisen und die bisher nicht in der Lage waren, das Rauchen zu beenden. Daraus könnten sich gezielte Präventionsansätze entwickeln. Es handelt sich um die bisher größte Fall-Kontroll-Studie zu den oben genannten Tumoren, mit insgesamt 2.338 Patienten und Patientinnen und einer vergleichbar großen Kontrollgruppe. Die Studie erstreckt sich über zehn europäische Länder, um eine weites Spektrum unterschiedlicher Lebensstile und Ernährungsformen untersuchen zu können. Alle Fälle und Kontrollen werden zu Alkohol und Rauchen, Ernährung und anderen Faktoren persönlich befragt.Von allen Teilnehmern und Teilnehmerinnen werden Blut- und Serumproben gewonnen, sowie Gewebeschnitte für die Diagnosesicherung. Mikroarray-Techniken kommen zum Einsatz, um 166 so genannte SNPs (single nucleotide polymorphisms) auf 51 verschieden Genen im Zusammenhang mit den genannten Tumoren zu untersuchen. Es handelt sich um Gene, die mit der Metabolisierung von Alkohol und Tabakrauch zusammenhängen, die mit Nikotinabhängigkeit in Verbindung stehen sowie um Tumorsuppressor- und DANN-Reparaturgene. Unter Berücksichtigung der Ein- und Ausschlusskriterien für Fälle und Kontrollen wurden aus Bremer Krankenhäusern insgesamt 292 Fälle mit der Lokalisation Mund-Rachen-Larynx und 328 Krankenhauskontrollen (ohne bösartige Erkrankungen) für die Studie rekrutiert. Auch 15 Jahre nach Ende der offiziellen Förderung des Projekts, bilden die Daten eine wertvolle Basis, auf deren Grundlage weiterhin wissenschaftliche Publikationen entstehen.
